If you donate to the Offertory online or in person in our church and pay more than this amount in tax, please consider filling in this Gift Aid form. It allows us to claim an additional 25% on your donation from the tax man. Your address is needed to identify you as a current UK taxpayer. Please let us know if you change your address, or stop paying tax.
